How much would you charge for this sort of thing, for an e-book only?

To be honest, I don't have a set price. If I'm slow I can usually negotiate something both parties are happy with; when I'm busy I'm less inclined to negotiate.

Right now I'm halfway between slow and busy.

It doesn't matter if it's eBook only, I still create it full size and high-resolution. It doesn't cost any more to make it print-compatible and it's better to start there and re-size it down for digital applications.

Usually what I do with design projects is rough-out two or three thumbnail covers just to get the idea across. Once the client approves a direction, I move to tighter art, with proofs at major stages along the way.

If the client already has an idea he/she wants me to consider, I certainly will, but I also try to come up with ideas they may not have considered because it's my job to surprise you and the reader.

I don't know what the going rate is for covers. I don't try to undercut the competition; I don't care what others charge. I'd rather set a price I think is fair and see how the client reacts. If we can reach an agreement, that's great, but I'm not afraid to (politely) walk away from a bad deal. I know clients want to get the biggest bang for the buck, and I don't blame them, but I think my work as a designer / illustrator has a proven value and so I charge a reasonable amount for my talent and experience.

That's the long answer. The short answer: "Let's talk."
