Proper Manuscript Format

I was wondering if there was a book or something available that could help one learn the perfect manuscript format. I already know that you need to have it double spaced and have your name on every page along with word count rights offerend ect, but I was thrown for a loop when I heard you can't use Itailcs and they have to be underlined.

Since there seem to be more rules to manuscripts than the writer's market shows, I was wondering if there was a book on how to format the perfect manuscript.

thanks in advance for any help or advice with this.

here's the one i use and recommend to mentees:

http://www.shunn.net/format/

it's much more detailed and bill offers format guides for novels and poems, as well as non-fiction... plus templates for both ms word and wordperfect users...
