I say language introduced to different cultures followed by repeated mispronunciation over a long period of time causes this.

As for how to use your quotation marks, let them hug the words not the punctuation if it is a comma e.g.
"love".

Where do you place the punctuation marks in relation to the quotation marks?

Depends on what style manual you use. Just as long as a writer picks one and is consistent with it, I don't worry about it terribly much.
By the rules I was taught, your example would be correct with the comma outside the quotation marks, (And I was educated in America!) because the comma does not apply to what is quoted. If it had been a complete sentence quoted, then the punctuation would be inside the quotes.

Thanks. I take it then that the punctuation at the end of the sentence follows the same rule.
But what about where it isn't just a single word being quoted? Where we have a full phrase of direct speech? For example, is it
John said, "It looks like rain today." or John said, "It looks like rain today". ?

If a complete sentence is quoted, then the final punctuation goes inside the quotes. At least according to most style manuals.
Quotation Marks
This is one of my favorite grammar reference sites. It is clear, concise and won't make you want to pour acid on your eyeballs.

Although I am American, I follow the English rule. The American style is a bastardization which should be rejected.
Also, it offends my delicate sensibilities. the punctuation looks like an interloper who has surreptitiously slipped inside the quotes that were not intended for it.
