Grammar questions

1) Do you hyphenate "pizza face" if it is used as a nickname?

2) Do you hyphenate "cliff diver" when used with an apostrophe to form a possessive noun? IE: the cliff-diver's sandwich.

3) Is this grammatically correct?

The blanket lay on the ground, covering a hole and held in place by large stones on each corner; the braiding spelled out "Welcome".

4) How about this one?

In what can only be described as a surprise development, he ran into the door -- still holding the kettle -- and with the grace of a of a dizzy penguin, he bounced back onto the floor.

In British English, the answer to all four questions is "yes".

I'm not sure about the hyphenations in US English.

...not necessarily, but you could, i suppose... look at the old army slang term, 'dog face'... it's not usually hyphenated...

2) Do you hyphenate "cliff diver" when used with an apostrophe to form a possessive noun? IE: the cliff-diver's sandwich.

...yes...

3) Is this grammatically correct?

The blanket lay on the ground, covering a hole and held in place by large stones on each corner; the braiding spelled out "Welcome".

...i wouldn't use the semicolon, would finish the sentence and start a new one... and in the us, the period and comma always go inside the " "...

4) How about this one?

In what can only be described as a surprise development, he ran into the door -- still holding the kettle -- and, with the grace of a of a dizzy penguin, he bounced back onto the floor.

...aside from the missing comma, it's ok... but i'd dump the last 'he'...
