should read 'page 2' if the first page has the title and beginning of the text... if the first page is just the cover page, move that to a separate file named 'cover page' w/ the title included... then, the first page that contains the title and the opening text will be 'page 1' and should have no header... to set that up, you go to 'page setup' in the 'file' menu... click on 'layout' and select 'different first page'... that will make the header not appear on the first page and your numbering will still begin with page #2 on the second page...

...the quote should just be typed above the beginning of the book... it shouldn't be on a page of its own, unless it's so long, it takes up a whole page... which is not a good idea... be sure you have permission to use the quote, if it's not in the public domain...

...why are there two of essentially the same thing?... or, did you mean 'acknowledgements' or something other than preface/introduction, which are merely different names for the same thing?... as are 'prologue' and 'foreword,' although the latter is often written by someone other than the author...

Most online manuscript format articles don't even mention adding in a table of contents. Are they not saying to put one in because they assume the author will place one in on their own?

...all books don't
have a toc... is this a novel?... or non-fiction?... if it's non-fiction, then you'll probably need one... are you sending this to a pod or other type of vanity press, or to a paying publisher?... that makes a major difference, since you do have to arrange your book the way you want it to be printed, if self-publishing, but do not do that, if you are dealing with a publisher who will pay you to publish your work... they have departments that take care of styling the book, and you are only to send them the ms in proper
reading form, not as it will appear in print...
