if we're talking beyond the written word, me and my friends use a dictaphone to record our crazy jokes at lunch, because we'll forget them and they dont have sticking power on paper (we have a very Pythonite humour)

so thats another idea. Especially if you know exactly how you want something to sound as well as appear when you come up with an idea, but at the time can't describe the sound exactly

I start out in a notebook, handwriting. But I keep it fairly short and choppy. Usually just the start of a chapter or half of one.
Then the next day, I copy it into the computer, taking more time to build detail and being more careful with words and sentence structure. Usually, by the time I'm finished copying it, I'm so engrossed in it I don't need to bother with distractions.
Starting out on the computer is hard for me. For some reason a blank sheet of paper is less threatening than a blank computer screen.
