When it's important for the audience to know exactly where and/or when a scene is taking place. In an establishing shot there will be a title on the bottom of the screen like:
Philadelphia, 1776
How would that be formatted?

I would say your best bet is
TITLE OVER:
Philadelphia, 1776
I'd also suggest downloading and referring to the Nicholl format sample

A street in colonial Philadelphia. Nice old homes, gas lamps.
A carriage approaches.
TITLE OVER: PHILADELPHIA, 1776
The carriage stops, JOHN PAINE steps out and holds up a hand for his SLUTTY DRESSED COMPANION.
But maybe like
EXT. JFK AIRPORT --DAY
STOCK SHOT: Plane lading at Kennedy Airport
TITLE OVER: JOHN F. KENNEDY AIRPORT, NEW YORK
I wouldn't worry about centering it.
Seems to me it's like any SUB, CARD, etc. A tag aligned left in caps.
